EYSC Recreational Registration Page
Welcome to the Eureka Youth Soccer Club (EYSC) Registration web page. The following sections apply to those players wanting to play Recreational Soccer. For those players that complete tryouts and are selected to play on a “Competitive” team, you will be provided the Registration packets by the Coach. The Coach will be responsible for submitting the completed team packets to the Select Registrar. Whether the player is on a Recreational or Competitive team, the EYSC requirements and items needed for registration are the same. Please note that the registration fees for the 2012 season have remained the same and are as follows:
Recreational Soccer: $ 90.00/player Competitive Soccer: $ 125.00 / player
Registration Process (After April 26th)

We have concluded the 3 advertised Walk-In registration sessions.

Mail-In registrations will end on June 30th. Registrations received after June 30th will be placed on a Stand-By list for possible placement on a team. Placements are limited to those vacancies on formed teams.

Stand-By Registrations will not be guaranteed a place on an EYSC team. All Mailed-In Registration Packets should be mailed to:

The following Registration Form is available below and is required to be printed and signed in duplicate.
2012 EYSC Registration Form
(please print and sign 2 copies for each player)
All Returning Players to EYSC should submit:
· 2 completed and signed EYSC Registration Forms (provided at the Registration session or on-line at the link above). If the on-line version is used, you must complete and sign two (2) originals for each player.
· A 1" x 1" color “head shot” photograph of your player (no hats please) for U10 and older. No photo needed for U6-U8 players.
· Cash or check for your registration fee of $90.00 / player. Checks should be made out to EYSC with the players name in the memo line of the check. We do not accept credit cards.
All New Players to EYSC should submit:
· A copy of their certified birth certificate for EYSC to keep. Hospital and Baptismal certificates will not be accepted for new players.
· A completed and signed EYSC Registration Form (provided at the Registration session or on-line at the link above). If the on-line version is used, you must complete and sign two (2) originals for each player.

* The player must be 5 by 12/31/12 to play in the U-6 age group.
** Example 1: A player born in July of 2000 will play in the U-14 bracket
** Example 2: A player born in August of 2000 will play in the U-12 bracket

Stand By Registration:
EYSC will attempt to place Stand-By Players on a team but the number of slots will be limited to openings for dropped players after the June 30th deadline. All players on the Stand-By list will be contacted by the respective Age Group Coordinator (AGC) as soon as a placement can be made. Please note that registrations received in June, July and August may not be accepted since team may already be formed.
Additional Notes on Team Formation:
In regard to building Recreational teams, EYSC will attempt to meet all Registration Requests in the following order.
· Returning players that Register at one of the three sessions.
· Coaches request for up to 4 players.
· Age pure teams
· Player requests
· Schools
Please note that it is intended for U6 teams to have an even amount of boys and girls if possible on each team. EYSC is not permitted to have a full team of only boys or a full team of only girls.
If you made it to a Registration Session and/or registered by the May 15th deadline, your forms will be processed first. All Players placed on a team will be contacted by your assigned coach in late July for Team Training beginning the first week of August. For players registered after May 15th or those players with an incomplete registration packet or on a Stand-By List, you will be notified by the Age Group Coordinator of your age bracket regarding placement on a team if an opening becomes available. The best way to ensure that your Registration is complete and you are guaranteed a team is to attend one of the 3 Registration Sessions. We highly encourage you to register in person. Please be patient while we take the time to make your soccer registration experience the best we can. Please bring all materials listed above to the registration session.
For information on registration or stand-by placements, we ask that you contact the Age Group Coordinator (AGC) for the age group of the player. Your respective AGC contact is provided on the Recreational web page. Please note while the Registrar is responsible for properly documenting and maintaining player and team information, it is the AGC that oversees team rosters, placement of players on a team, and team requests. Specific questions relating to team formations should always be directed to your respective AGC.
Uniforms:
EYSC is using the same style uniform as used during the 2011 season. For more information on the Recreational uniform, please see the Uniform Page of the EYSC website.
Refunds:
While it is our intention to only collect fees for players placed on a team, EYSC does incur costs during the registration process. We have League expenses, club t-shirt expenses, field expenses, etc...that must be paid at or after the Registration period but before the first training begins. Based on these fees, a full and prorated refund policy is utilized by the Club. The date of the refund amount is based on the date that the registration Refund Form is received. To request a registration refund, please read the policy and complete the attached refund form. It can then be mailed to the following address:

Please note the refund policy provided on the form, as refund requests that fall outside of the policy will not be honored. Questions relating to player fees, payments, or refunds should be directed to the Registrar at eperjanik@eysc.com.
